What to bring to your first visit?

We work hard to ensure that your child receives prompt medical care. To help expedite the first appointment, we ask that you please arrive a few minutes early and bring your child's insurance card and previous medical records, including immunization records.  Be prepared to complete paperwork regarding your family medical history, your child's medical history, and insurance information.

NEWBORNS

We understand this can be a very busy time in your life, but please remember to enroll your baby with your insurance as soon as possible and no later than 30 days after your baby’s birth date. Your baby is usually covered by the mother’s insurance for the first 30 days of life, but there are exceptions,  so please verify with your insurance. If the baby is not enrolled within 30 days, then the services rendered on day 31 and after will NOT BE COVERED and you will be asked to PAY AT TIME OF SERVICE.

In order to enroll your new baby with your insurance, you usually have to contact your employer or your partner’s employer to fill out the appropriate forms. It often takes some time to process all the paperwork. Hence, you should do this as soon as possible in order to ensure that your baby will be covered.

After you enrolled your baby, please provide us a copy of your baby’s health plan ID card for our records
